Texas has not expanded eligibility for Medicaid under the Affordable Care Act (ACA). As a result, Texas has the biggest coverage gap in the country, with an estimated 771,000 residents ineligible for Medicaid and also ineligible for premium subsidies to offset the cost of private coverage in the exchange.
More than 4.4 million people in Texas have Medicare plans – a little more than 14% of the state’s population. Nearly half of Texas Medicare beneficiaries have Medicare Advantage plans. Medicare Advantage plan availability ranges from 12 plans to 108 plans, depending on the county.
The insurance marketplace offers health insurance plans for individuals and families who need to buy their own health insurance. This includes early retirees (who will transition to Medicare when they turn 65), self-employed residents, and people employed by a small business that doesn’t offer employer-sponsored health benefits.
Using the marketplace is optional; individuals and families in Texas can choose instead to buy coverage directly from a health insurance company. But the marketplace is the only place where Texas residents can qualify for financial a-sistance benefits based on their household income. This includes premium subsidies (which offset the monthly cost of coverage) and cost-sharing reductions (which reduce out-of-pocket medical costs — deductibles, coinsurance, and copays).

Enrollment reached a record high in 2022, with more than 1.8 million enrollees. Aetna/CVS, Bright Health, Moda, and UnitedHealthcare joined the Texas exchange for 2022, after two other insurers (Friday and Scott & White Health Plan) joined the exchange for 2021. Texas will have its own rate review process as of 2022.
Fourteen health insurance carriers offer 2022 coverage through the Texas marketplace/exchange. At least two new insurers (Cigna and Ascension Personalized Care) are joining the Texas individual/family health insurance market for 2023. It’s expected that their plans will be available through the marketplace in some areas, which would bring total insurer participation in the Texas marketplace to 16.
Texas uses the federally run exchange at HealthCare.gov, and the state has taken a very hands-off approach with regards to implementing the ACA. Texas has not expanded Medicaid, and through 2021, was one of just three states that left the rate review process for ACA-compliant plans to CMS. That has changed as of 2022, however, with the implementation of a state-based rate review program (details below).
Texas has one of the highest exchange enrollments in the country. 1,840,947 people enrolled in private plans through the Texas exchange during the open enrollment period for 2022 coverage, which far surpassed any previous year.
Rates increased sharply in 2018, and insurers started adding the cost of cost-sharing reductions (CSR) to silver plan rates, resulting in particularly large premium subsidies (which are based on the cost of silver plans) in 2018. For 2019, however, the average rate increase was much more modest, at just over 2%. For 2020, average rates decreased slightly. And although the average rate increase was still in the single digits for 2021, it was a larger rate increase than we saw across most states. For 2022, the average rate increase in Texas was in the low single digits. The cost of CSR is still being added to silver plan rates, so subsidies are still disproportionately large in Texas. And the American Rescue Plan has made subsidies larger than ever for 2021 and 2022.
According to US Census Bureau data, Texas had the highest uninsured rate in the country in 2013, at 22.1%, and it has continued to have the highest uninsured rate in the nation ever since. Although 18.4% of Texans were still uninsured as of 2019, a substantial number of Texas residents have obtained health coverage since the ACA was implemented.

Texas sponsors two programs to help families pay for insurance: the Children’s Health Insurance Program (CHIP) and Health Insurance Premium Payment (HIPP) program.
Children in Texas without health insurance may be able to get low cost or free health coverage from the CHIP or Children’s Medicaid. Families with children who get Children’s Medicaid pay nothing and children with CHIP pay no more than $50 a year for health care coverage.
HIPP helps pay health insurance payments for families that have at least one person who gets Medicaid and can get insurance through a family member’s work. Your family can pick from more doctors and has more choices than with Medicaid alone.

If you wish to receive services at Texas Health as an out of network hospital, then a single case agreement (SCA) may be required. A single case agreement (SCA) is an agreement between Texas Health and a non-contracted insurance or other coverage plan. The SCA outlines authorization and payment terms prior to services performed. A patient can also request a single case agreement (SCA) directly from his/her insurance or other coverage plan if they want to have services at Texas Health. The insurance or other coverage plan would work directly with Texas Health to complete the SCA prior to services performed.

Typically, most insurance plans do not pay 100% of healthcare expenses except for specific preventive care services. The amount of coverage depends on the type of policy. There are plans for individuals and plans that are offered by employers - HMO (Health Maintenance Organization), PPO (Preferred Provider Organization) and high deductible plans. Each type of plan has specific coverage, benefits, exclusions, and limitations, so it is important to understand how your policy works for you. Texas A&M University students are not required to have insurance to utilize SHS. If the student has health insurance, SHS will collect the information and file with the insurance company. A copy of the insurance card will be required at the time of check in. Any balances not paid by the student’s insurance carrier will be transferred to the student’s fee account at Student Business Services. A verification of benefits does not guarantee payment.
SHS does not contract with insurance companies, managed care networks, or government insurance programs. However, depending on the patient’s insurance plan, a majority of our charge will partially get reimbursed. SHS cannot file pharmacy claims unless the student is covered under one of the TAMU Sponsored Plans.
Student Health Services collaborates with a Texas A&M University-sponsored student health insurance plan to provide optimal healthcare coverage for TAMU students at the A.P. Beutel Health Center. This health insurance plan provides students excellent coverage for the majority of services rendered at SHS.
Texas A&M University students who are enrolled in at least nine (9) hours are eligible to enroll in the University Sponsored plan. Students who are enrolled in special classes and are taking less than nine (9) credit/contact hours will be determined eligible to enroll in the University Sponsored plan if the coursework meets the criteria for the completion of a degree plan or international program as defined and approved by The Texas A&M University System. Graduate students taking at least five (5) credit hours of classes are eligible to enroll in this insurance plan. Medicare Advantage boosterism & a health insurance firewall

We are at the point in the Medicare Advantage Discourse where health insurers have convinced themselves, and are continuing to try to convince the public, that MA saves taxpayers oodles of money. The verdict from experts on this extremely in-the-weeds issue: Don’t buy it.
The latest exhibit comes from America’s Health Insurance Plans, which released a report last week that was conducted by actuaries at Wakely Consulting Group. AHIP claims the report proves Medicare Advantage — the controversial alternative to original Medicare that has become a cash cow for insurers — is “saving Americans billions of dollars every year.” The actuaries, however, never use that language in the report.
STAT spoke with several independent Medicare policy experts, all of whom said AHIP’s report was incomplete at best and refuted by other studies that analyzed the same data. Analysts also stressed the federal government certainly isn’t reaping any savings from Medicare Advantage.

The Texas Medical A-sociation has filed a brand new lawsuit in a district court in eastern Texas, arguing the Biden administration’s final surprise billing rule from August still puts a “thumb on the scale” for health insurers. Three dominant provider lobbying groups — the American Hospital A-sociation, American Medical A-sociation, and A-sociation of Air Medical Services — also told a judge last week they were dismissing their claims against the No Surprises Act in their D.C. district court and instead were going to cheer on their Texan comrades’ new lawsuit.
