Texas has not expanded eligibility for Medicaid under the Affordable Care Act (ACA). As a result, Texas has the biggest coverage gap in the country, with an estimated 771,000 residents ineligible for Medicaid and also ineligible for premium subsidies to offset the cost of private coverage in the exchange.
More than 4.4 million people in Texas have Medicare plans – a little more than 14% of the state’s population. Nearly half of Texas Medicare beneficiaries have Medicare Advantage plans. Medicare Advantage plan availability ranges from 12 plans to 108 plans, depending on the county.
The insurance marketplace offers health insurance plans for individuals and families who need to buy their own health insurance. This includes early retirees (who will transition to Medicare when they turn 65), self-employed residents, and people employed by a small business that doesn’t offer employer-sponsored health benefits.
Using the marketplace is optional; individuals and families in Texas can choose instead to buy coverage directly from a health insurance company. But the marketplace is the only place where Texas residents can qualify for financial a-sistance benefits based on their household income. This includes premium subsidies (which offset the monthly cost of coverage) and cost-sharing reductions (which reduce out-of-pocket medical costs — deductibles, coinsurance, and copays).

Depending on the health insurance company, Texas individual and family health insurance rates for couples can be based on the age of the youngest person on the policy. Some couples with an age difference of several years may find they save money by applying for the same plan rather than individually. Couples that are only a few years apart might be able to reduce their total costs by applying for separate health insurance plans. If you decide to apply for a health insurance plan as a couple, make sure the plan you choose covers the benefits both of you need.

Texas residents are not required to have health insurance under state law. However, the Affordable Care Act does mandate a health insurance requirement on a federal level that includes Texans. Texas utilizes the federal exchange for health plans and has one of the highest enrollment rates in the country.
Texas open enrollment for 2020 plans has ended, unless an individual has a qualifying event. The open enrollment for 2021 plans will commence on November 1, 2020. Plans are available to residents through the federal exchange and private insurance companies.
Texas health insurance marketplace 2022 guide | healthinsurance.org
Enrollment reached a record high in 2022, with more than 1.8 million enrollees. Aetna/CVS, Bright Health, Moda, and UnitedHealthcare joined the Texas exchange for 2022, after two other insurers (Friday and Scott & White Health Plan) joined the exchange for 2021. Texas will have its own rate review process as of 2022.
Fourteen health insurance carriers offer 2022 coverage through the Texas marketplace/exchange. At least two new insurers (Cigna and Ascension Personalized Care) are joining the Texas individual/family health insurance market for 2023. It’s expected that their plans will be available through the marketplace in some areas, which would bring total insurer participation in the Texas marketplace to 16.
Texas uses the federally run exchange at HealthCare.gov, and the state has taken a very hands-off approach with regards to implementing the ACA. Texas has not expanded Medicaid, and through 2021, was one of just three states that left the rate review process for ACA-compliant plans to CMS. That has changed as of 2022, however, with the implementation of a state-based rate review program (details below).
Texas has one of the highest exchange enrollments in the country. 1,840,947 people enrolled in private plans through the Texas exchange during the open enrollment period for 2022 coverage, which far surpassed any previous year.
Rates increased sharply in 2018, and insurers started adding the cost of cost-sharing reductions (CSR) to silver plan rates, resulting in particularly large premium subsidies (which are based on the cost of silver plans) in 2018. For 2019, however, the average rate increase was much more modest, at just over 2%. For 2020, average rates decreased slightly. And although the average rate increase was still in the single digits for 2021, it was a larger rate increase than we saw across most states. For 2022, the average rate increase in Texas was in the low single digits. The cost of CSR is still being added to silver plan rates, so subsidies are still disproportionately large in Texas. And the American Rescue Plan has made subsidies larger than ever for 2021 and 2022.
According to US Census Bureau data, Texas had the highest uninsured rate in the country in 2013, at 22.1%, and it has continued to have the highest uninsured rate in the nation ever since. Although 18.4% of Texans were still uninsured as of 2019, a substantial number of Texas residents have obtained health coverage since the ACA was implemented.

Shopping for coverage on your own? Open Enrollment for 2022 plans has ended. If you've had a qualifying life event you could still be eligible to get a plan during Special Enrollment, which runs until Oct. 31, 2022.
During annual Open Enrollment (November 1-January 15, in most states). This applies to new customers, and to current customers who want to renew or make changes to their existing plan—you can shop here on Cigna.com.
During the Special Enrollment Period (January 16-October 31) you can shop for a plan on HealthCare.gov. If you've had a qualifying life event, you may be eligible to get a plan during this time. Life events could include marriage, new child, recent move, and more.
Notice for North Carolina residents: Your actual expenses for covered services may exceed the stated coinsurance percentage or copayment amount because the actual provider charges may not be used to determine plan and member payment obligations.
Individual & Family Health Insurance Texas | Stateside Insurance Services
We Focus on Making Health Insurance Affordable & Easy for Texans. See our available individual & family health insurance plans in Texas.
Individual and family health insurance plans cover expenses in the case of illness, serious medical emergencies and help you and your family stay on top of preventative healthcare services. Health Insurance coverage can save you money on doctor’s visits, prescription drugs, preventative care and other healthcare services. Typical health insurance plans for individuals in addition to monthly premiums include costs such as an annual deductible, copayments and coinsurance.
You may know Affordable Care Act (ACA) plans as major medical or even “Obamacare,” but whatever you call them, both On and Off Exchange plans meet all the requirements of the ACA. No matter which Exchange you shop, the prices of health insurance will always remain the same. The main difference is access to financial a-sistance, known as a subsidy.
Off-Exchange plans are offered privately by each health insurance carrier within the state. These plans offer the same Essential Health Benefits, complaint to the ACA, the only difference is Off Marketplace does not offer subsidies.
ACA plans provide benefits for a broad range of health care services, both inpatient and outpatient, and can save you money on routine doctor visits, prescription drugs, preventative care, hospital stays, and more.
If you can’t afford an Affordable Care Act (ACA) plan or have missed the cutoff to apply, you may want to consider a short-term plan. Compared to ACA plans, short-term health insurance typically provides coverage limitations not found in the ACA, does not provide coverage for pre-existing health conditions and the applicant will be subject to underwriting. But short-term plans do accept applications year-round, and they can help offset costs if you have a medical emergency unrelated to a preexisting condition.

Getting sick can be expensive. Even minor illnesses and injuries can cost thousands of dollars to diagnose and treat. Major illnesses can cost many times that. Health care coverage helps you get the care you need and protects you and your family financially if you get sick or injured.
Insurance you get through your job or an a-sociation is called group insurance. You must be a member of the group to get coverage. Most people get health insurance through their job, but not all employers offer it.
Insurance you buy directly from a company or the marketplace is called individual insurance because it’s sold to individuals, not to a group. For lists of companies and health maintenance organizations (HMOs) that sell individual health insurance in Texas, visit the Companies that sell health plans page on our website.
You can keep your dependent children on your plan until they turn 26. They don't have to live at home, be enrolled in school, or be claimed as a dependent on your tax return. You can keep married children on your plan, but you can’t add their spouses or children to it.
The open enrollment period for marketplace and individual plans is from November 1 to December 15 each year. You can buy at other times only if you lose your coverage or have a life change. Life changes include things like getting married or divorced, having a baby, or adopting a child.

Health Insurance Plan
In 2022, the cheapest health insurance in Texas is from Blue Cross and Blue Shield.
The cheapest health insurance plan in Texas varies by county. In 37% of Texas counties, the cheapest Silver plan is the Blue Advantage Silver HMO. Another widely available cheap option is the FirstCare Prime Silver HMO. You can find cheap, subsidized policies on the Texas health insurance exchange from up to 10 health insurance companies, although not all companies offer plans in every county.
To help you get started finding the best health insurance plan available for your preferred level of coverage, we compared Texas policies by plan tier and identified the cheapest health insurance option available in the state.
The set of insurers and health plans varies by county, so not all of those listed above are available in every region. We recommend using these as a starting point to a-sess the monthly premiums you can expect as compared to benefits and out-of-pocket expenses.
Consumers with higher medical needs, especially those who need routine prescriptions or expensive procedures, should choose a plan with better coverage such as a Gold-tier plan. These plans have a higher monthly cost, but they will reduce your out-of-pocket expenses for medical care, ultimately saving you money.
For households with incomes less than 250% of the federal poverty level, a Silver plan is almost always the best option. These will offer lower premiums than Gold plans, and their cost sharing will be adjusted to match more expensive options.

Finding the right group health plan for your business can be downright intimidating: sorting through lists of insurance companies and plans; checking and re-checking the dollars and totals for deductibles and co-pays; making sense of plan limitations and exclusions; deciphering a dictionary’s worth of insurance-speak. It’s enough to make anyone feel like a high-school freshman again.
Texas insurance law allows a wide array of health care coverage plans and packages. All group health insurance has its limitations and finding the right employee health plan at the right price can be challenging.
In Texas, the term “small employer” is a special insurance designation reserved for businesses with two to 50 eligible employees. The law provides some added protections to these businesses, including a 15 percent annual cap on rate increases due to health factors, a state-enforced guarantee that carriers cannot arbitrarily discontinue coverage, and a cooperative purchasing provision that lets small employers pool their purchasing clout to negotiate lower rates.
For employees of small businesses in Dallas, Houston and throughout Texas, the law provides several ways to maintain benefits after leaving a job and limits the waiting period before pre-existing conditions are covered.
Texas businesses with two to 50 eligible employees may obtain small-employer coverage from either a traditional insurance company or a health maintenance organization (HMO). Eligible employees are defined as those who usually work at least 30 hours per week; are not classified as temporary, part-time, or seasonal; and are not already covered by another group health plan. A business’ owners count toward the employee total.
