Texas has not expanded eligibility for Medicaid under the Affordable Care Act (ACA). As a result, Texas has the biggest coverage gap in the country, with an estimated 771,000 residents ineligible for Medicaid and also ineligible for premium subsidies to offset the cost of private coverage in the exchange.
More than 4.4 million people in Texas have Medicare plans – a little more than 14% of the state’s population. Nearly half of Texas Medicare beneficiaries have Medicare Advantage plans. Medicare Advantage plan availability ranges from 12 plans to 108 plans, depending on the county.
The insurance marketplace offers health insurance plans for individuals and families who need to buy their own health insurance. This includes early retirees (who will transition to Medicare when they turn 65), self-employed residents, and people employed by a small business that doesn’t offer employer-sponsored health benefits.
Using the marketplace is optional; individuals and families in Texas can choose instead to buy coverage directly from a health insurance company. But the marketplace is the only place where Texas residents can qualify for financial a-sistance benefits based on their household income. This includes premium subsidies (which offset the monthly cost of coverage) and cost-sharing reductions (which reduce out-of-pocket medical costs — deductibles, coinsurance, and copays).

There are two paths to health insurance for Texans: private and government-funded. Deciding which works best for you depends on factors, such as your age, income, health condition, or whether or not you own a business.
If you’re under 65 and don’t qualify for Medicare, your best option for affordable insurance may be an Obamacare plan. You may also consider another type of comprehensive low-cost plan that doesn’t qualify for ACA subsidies (like a catastrophic health plan) if you are under the age of 30. Depending on your income, Medicaid could serve as an option. (See below for more information).

Getting sick can be expensive. Even minor illnesses and injuries can cost thousands of dollars to diagnose and treat. Major illnesses can cost many times that. Health care coverage helps you get the care you need and protects you and your family financially if you get sick or injured.
Insurance you get through your job or an a-sociation is called group insurance. You must be a member of the group to get coverage. Most people get health insurance through their job, but not all employers offer it.
Insurance you buy directly from a company or the marketplace is called individual insurance because it’s sold to individuals, not to a group. For lists of companies and health maintenance organizations (HMOs) that sell individual health insurance in Texas, visit the Companies that sell health plans page on our website.
You can keep your dependent children on your plan until they turn 26. They don't have to live at home, be enrolled in school, or be claimed as a dependent on your tax return. You can keep married children on your plan, but you can’t add their spouses or children to it.
The open enrollment period for marketplace and individual plans is from November 1 to December 15 each year. You can buy at other times only if you lose your coverage or have a life change. Life changes include things like getting married or divorced, having a baby, or adopting a child.

Medicaid is a federal and state partnership, a jointly funded a-sistance program that helps citizens of the United States (or other specific qualified non-citizens, like lawful permanent residents) with limited income and resources to afford health insurance. Created in 1965, the Medicaid public insurance program provides free or low-cost health coverage to millions of people, including certain eligible low-income Americans, some families and children, pregnant women, elderly individuals, and people with disabilities. It also provides health benefits to many individuals who are not covered by Medicare, the national health insurance program of the United States, such as those in nursing homes, and those requiring personal care services. Medicaid programs are required to follow federal guidelines, though they are administered at the state level; program eligibility and health coverage specifics (and a-sociated costs) can vary significantly between states.
Medicaid is available to certain individuals who are eligible as determined by federal and state law. In some states, Medicaid has been expanded in recent years to provide health coverage for all adults with an income level below a specified minimum. Medicaid-insured parties are usually not held financially responsible for medical expenses covered under the program, though a small copayment is sometimes required, depending on the rules of the state in which those medical services were rendered. Some states allow for Medicaid programs to sends payments directly to healthcare providers when billed for services on behalf of any Medicaid-insured individual, while others utilize private insurance companies to provide the Medicaid coverage required.
States establish and administer their own Medicaid programs. Each state determines the types of services covered, as well as the amount/duration, and the scope and scale of those services within federal guidelines and requirements. Federal law requires states to provide certain mandatory benefits to Medicaid recipients; but individual states are given the purview to determine which, if any, optional benefits they will provide. Mandatory benefits include a wide range of services such as inpatient and outpatient hospital services, laboratory and x-ray services, nursing and home health services, nurse-midwife and birthing center services, and several others. Optional benefits that each state can choose to provide include prescription drugs, physical and occupational therapy, respiratory care services, dental services, hospice care, and more. (The Medicaid website provides a complete list of both mandatory and optional benefits.)
Medicaid is sometimes confused with Medicare (the federally administered, federally funded health insurance program for people over 65 and some people with disabilities), and for good reason. there is some overlap between the Medicaid and Medicare that allows for dual eligibility for millions of low-income seniors and people with disabilities. The Centers for Medicare and Medicaid Services (CMS) state that while Medicare and Medicaid cover many of the same services, Medicare is the first payer for any Medicare-covered services that are also covered by Medicaid, making Medicaid the “payer of last resort;” and Medicaid pays for any services not covered by Medicare (e.g. long-term interventions or treatments). CMS works to make sure that dually eligible people have access to the highest quality healthcare possible as cost-effectively as possible.
