4 health insurance plans are included in the Affordable Care Act. Find out which plans help to lower out-of-pocket costs, deductibles, and co-payments.
3. Coinsurance: The percentage of the costs of a covered healthcare service that you are responsible for. An example: if the coinsurance is 20%, and your health plan allows $100 for an office visit and you have paid your deductible, your coinsurance payment will be $20 (i.e., 20% of $100). The insurance plan pays the rest.
4. Copayment: A fixed amount that you pay for a specific, covered, healthcare service. This is usually paid at the time of service. For example, when you go in for a doctor’s visit, you pay $15 at the time of the visit. The amount varies.
5. Deductible: This is the amount of money you have agreed to pay for health services before your insurance company starts paying. For example, if the plan you have has a $1,000 deductible, the insurance plan pays nothing until you have paid $1,000 for services. Once you have paid for $1,000 worth of healthcare services (in one year), the insurance plan starts paying for additional services.
The general rule is that the lower the premiums, the higher the out-of-pocket costs when you actually need care, and vice versa—the higher the premiums, the lower the out-of-pocket costs when you require care.
Gold and Platinum levels will have lower deductibles, co-payments and co-insurance, but will probably have higher monthly premiums. Bronze and Silver plans will probably have lower premiums, but out-of-pocket costs are likely to be higher.

Employers must provide employees with a standard "Summary of Benefits and Coverage" (SBC) form explaining what their health plan covers and what it costs. Employees can't contribute more than the annual dollar limit set by the IRS to their Flexible Spending Accounts. That limit doesn't apply to employer contributions to the employees' FSAs. Plans on the health care Marketplace are categorized based on how you and your plan split the cost of your health care. They go by Bronze, Silver, Gold and Platinum — known as the “metal” categories. For 2022, UnitedHealthcare is offering plan options in the Bronze, Silver and Gold metal levels.
The health care Marketplace (also called the Exchange), is where Affordable Care Act (ACA) health care plans are sold. You may also hear names like ObamaCare, BidenCare and Individual and Family Plans. These are all health care plans that people can buy on their own, rather than through an employer or another government-run program, like Medicare or Medicaid.

The ACA included more than 1,000 pages of provisions intended to make coverage affordable for and accessible to millions of Americans who struggled to pay for individual coverage – many of whom could not buy individual coverage at any price due to pre-existing medical conditions.
The law sharply reduced the number of uninsured Americans. And although the uninsured rate increased under the Trump administration’s watch, enrollment in marketplace plans reached an all-time high in 2021 (and 2022 enrollment is on track to be significantly higher), and so has Medicaid/CHIP enrollment.

Providing affordable healthcare to more individuals and families is possible through the ACA because of subsidies called premium tax credits. The subsidies lower the cost of health insurance for households with incomes between 100% and 400% of the federal poverty level (FPL). In 2021, households with an income of greater than 400% still qualified for the premium tax credit.
Premium tax credits are refundable, and they help low- or moderate-income individuals and families buy coverage through the Health Insurance Marketplace. As the IRS explains, your premium tax credit amount depends on household income. The marketplace computes your premium tax credit when you enroll.
